What Is Fibromyalgia?
Fibromyalgia is characterised by widespread musculoskeletal pain, fatigue, sleep disturbances, and often cognitive symptoms ('fibro fog'). The current understanding positions it as a condition of central sensitisation — a state in which the central nervous system becomes hyperresponsive to pain signals, amplifying them beyond their normal intensity.
Diagnosis is clinical, based on the 2016 ACR diagnostic criteria. There is no blood test or imaging finding that definitively confirms fibromyalgia. Diagnosis requires the exclusion of other conditions that could account for the symptoms — rheumatoid arthritis, lupus, thyroid disease, and others — making a thorough medical workup important.
Conventional Treatments
Pharmacological options for fibromyalgia have modest evidence at best. Duloxetine (an SNRI antidepressant), pregabalin, and milnacipran are the medications with the strongest evidence base and regulatory approval for fibromyalgia in various countries. They can meaningfully reduce pain in some patients but do not work for everyone and carry side effect profiles that limit their use in others.
Simple analgesics, including paracetamol and NSAIDs, have limited evidence for fibromyalgia pain. Opioids are generally not recommended and may worsen central sensitisation over time.
Non-Pharmacological Approaches With Evidence
Aerobic Exercise
Low-to-moderate intensity aerobic exercise is the most consistently evidence-supported treatment for fibromyalgia. Multiple systematic reviews confirm its benefits for pain, fatigue, and overall function. The challenge is that exercise intolerance is common in fibromyalgia — starting very gradually and increasing extremely slowly is essential to avoid flares.
Cognitive Behavioural Therapy (CBT)
CBT for chronic pain has good evidence for reducing pain catastrophising, improving function, and enhancing quality of life, even when it does not reduce pain intensity itself. It addresses the psychological amplification of pain that central sensitisation creates.
Mindfulness-Based Stress Reduction (MBSR)
MBSR has growing evidence for fibromyalgia. Its mechanism — reducing the stress and emotional reactivity that amplify pain signals — is well-matched to the central sensitisation model of fibromyalgia.
Hydrotherapy and Pool Exercise
Warm water exercise is particularly well-tolerated in fibromyalgia. The warmth reduces muscle tension and pain, and buoyancy reduces joint loading. Aquatic physiotherapy is a practical first exercise option for many patients.
Electrostimulation for Chronic Pain
Electrostimulation — both TENS and microcurrent — has been used in chronic pain management contexts, including fibromyalgia. The rationale is that electrical stimulation may modulate pain signal processing and support localised pain relief in affected areas.
